Assisted Travel Registration

 

 

 

 

Registration:

 

New campers or campers who have not attended camp in the last year

Step 1: Apply online here or call Kathy Blumke at (410) 323-0500 ext. 309 to receive a Registration Form and Skills Check List. Return the completed Registration Form and Skills List with a deposit for each program ($400 per assisted travel program) or a letter from a service organization stating the amount to be paid by them. Please note that deposits are non-transferrable and non-refundable.

Step 2: An Administrator will contact you to set-up and interview about your camper and to answer any questions about our program.

Step 3: Once the interview is complete, you will recieve a confirmation letter with a Medical Form and Medication Form (both forms are also available for download above). Both of these forms must be filled out by a doctor and must be received by The League at least two weeks prior to the program. If there are changes in your medication before the program begins, be sure to forward a new doctor's order to The League as soon as possible.

Step 4: Mail your final payments to The League by January 1st for all Assited Travel Programs. If your final payment or letter of promise is not received by The League before that date, you run the risk of losing your spot in the program.

Campers who attended camp in 2005:

Step 2: When you receive your confirmation letter, make sure tht all the forms from the summer of 2005 are still current. If a camper's medication changes from the original orders prior to the program, please send a revised order to The League as soon as possible.

Step 3: Mail your final payments to The League by January 1st for all Assited Travel Programs. If your final payment or letter of promise is not received by The League before that date, you run the risk of losing your spot in the program.

Assited Travel Refund Policy:

All changes and cancellations must be in writing. Changes or cancellations made at least 90 days before departure will receive a refund minus $400 and any pre-purchased portion of the vacation package. Trips canceled after the 90-day deadline will still require full payment. For reasons of safety, travelers may lose their space on the trip if necessary paperwork is not received by the due date. Change/cancellation penalties will apply. No refund for cancellations or delays due to weather. Changes to airport/departure and en route plans are considered a change in itinerary and penalties will apply. Traveler can be refused on the trip if they show up in ill health or without all medication and dosage for the trip plus one day. No refund will be issued if the traveler is refused boarding. Camping & Therapeutic Recreation reserves the right to determine sending a partcipant home, at the participant's expense, if a participant exhibits inappropriate behavior, is sick for more than 24 hours, or is not having a successful recreational experience. This is done at the discretion of the Director.
