Social Services to Adults

Builds on the strengths of the adult's community and family support system rather than replace them. Assistance adults with limited capacities seeking to remain or become self-supporting and self-sufficient; it seeks to prevent abuse, neglect or exploitation; and it reduces unnecessary institutionalization or secures appropriate institutional care when necessary.

Eligibility:
No fee. Functional disabilities due to advanced age 65+ without regard to income.Those under 65 may receive services provided their income does not exceed 80% of the State median income.

In-Home Aide Services (IHAS)

Offers aide services in the consumer's home to provide assistance with: dressing, bathing, transferring, rooming, eating, cooking, laundry, light cleaning, shopping and; allowing an adult with a disability to remain living at home and to avoid unnecessary or premature moves to nursing homes or other out-of-home placement. Provides ongoing case management to assist with planning and exploring other needed services.

Non-technical, medically-oriented care to allow recipients to be treated by their physicians on an outpatient basis
Developed for long-term maintenance of patients who are physically or mentally chronically ill or disable or frail elderly.
Transportation to medical appointments with an attendant
Household tasks incidental to home care
Monitoring activities for safety
Dressing, bathing, toileting, meal preparation and feeding, personal hygiene/grooming, ambulation

Designed to help seniors remain in their own homes by means of modification.

For example: grab bars, help get a chair lift /chair stair glider, make changes in the kitchen to help them to reach to become more senior friendly for them Loan closet to borrow assistive devices such as wheelchairs .
