Day Habilitation Program Assistant
Position: Community Learning Retention Specialist – Program Assistant
Department: Day Habilitation Services
Reports to: Manager, Day Habilitation
Exempt/Non Exempt: Non Exempt
Summary: To provide developmental/ vocational habilitation training services consistent with each participants Individual Plan by implementation of skill acquisition in areas of self- help, daily living skills, socialization, leisure, basic academics, motor skills, communication and behavior.
Essential Duties and Responsibilities:

Supervise, train and assist individuals of assigned group to meet his/ her goals.

Along with Program Manager, plan activities that address the goals set for in an Individual’s plan

Implement and record data pertaining to the Individual plan

Implement individual behavior programs.

Complete required data entry and documentation.

Ensure that the safety and health care practices are observed.

Assist all program individuals to keep work and training area clean and orderly/

Assist participants with community integration projects
-
Assist participants with toileting and personal hygiene.

Follow correct lifting procedures when helping non- ambulatory individuals move to different locations.

Maintain a positive and productive attitude.
-
Assist individuals with transportation through Mobility.
-
Provide transportation and assistance to clients for community appointments & field trips.

Complete transportation logs regarding daily mileage and inspection sheets.

Obey all traffic laws and communicate all violations and malfunctions, involving agency vehicles, to the Transportation Coordinator.

Attend meetings, conferences, and trainings as scheduled

Serve as an advocate for the program participants.

Assist supervisor in the timely completion of monthly billing.

Maintain up to date tracking sheets

Provide activities with an individual(s) as assigned.

All other duties as required

Must be able to assist with toileting, meals, and transfers of individuals needing assistance.

Learn and utilize ETO software.

Qualifications Requirements:

Must be able to pass a criminal background check and drug screening.

AA Degree in Human Services or related field/ and or 2 years experience working with people who have developmental disabilities and have completed high school, or possess GED.

Possess verbal and written skills to meet job requirements

Ability to complete training as required

Must be self motivated and able to work independently

Must be willing to meet at various partnered sites throughout the Baltimore area for the work shift

Clean driving record (which includes no more than 3 points)
-
Two years of driving experience, ability to drive up to 16 passenger van and access to a personal vehicle for participant transport as needed.
