Day Habilitation Group Leader
Summary:
This position provides pre-vocational and life skills training to adults with Developmental Disabilities consistent with each participants Person Centered Plan. This is done through verbal and physical direction, demonstration and setting a good example. Skills areas include self-help, daily living skills, safety skills, socialization, leisure, basic academics, motor skills, communication and behavior management. These skills will be taught in small groups at a variety of community settings.
Essential Duties and Responsibilities:

Support, teach and assist individuals of assigned group to meet his/ her goals, and provide line of sight supervision.

Along with Program Manager, plan activities that address the goals set forth in the Person Centered Plan.

Implement and record data pertaining to each participant's Person Centered Plan.

Implement individual behavior programs.

Complete required daily data entry and documentation.

Ensure that the safety and health care practices are observed.

Assist all program individuals in keeping work and training areas clean and orderly.

Assist participants with community integration projects such as working in volunteer jobs and recreational activities.

Assist participants with toileting and personal hygiene as needed.

Follow correct lifting procedures when helping non-ambulatory individuals move to different locations.

Maintain a positive and productive attitude.

Assist individuals with advocating for and arranging transportation through MTA Mobility.

Provide transportation and assistance to clients for community activities, including assistance with participants in wheelchairs using League vans.

Complete transportation logs regarding daily mileage and inspection sheets.

Obey all traffic laws and communicate all violations and malfunctions involving agency vehicles to the Transportation Coordinator.

Attend meetings, conferences, and trainings as scheduled

Serve as an advocate for the program participants.
Requirements:

AA Degree in Human Services or related field/ and or 2 years direct staff experience working with people who have developmental disabilities

Minimum education requirement is high school diploma or GED

Possess verbal and written skills to meet job requirements

Ability to complete training as required

Must be self-motivated and able to work independently

Current MD driver’s license and ability to drive up to 16 passenger vehicles with wheelchair lift

Clean driving record (which includes no more than 2 points)

Must be able to pass a criminal background check and drug screening.
