Health Services RN (Adult Medical Day)

Position: Health Services Nurse           

Department: Adult Medical Day  

Reports to:  Director, Health Services

Hours:  M- F 8am-4pm

 

Summary: The Health Services Nurse is responsible for monitoring the health status of all League participants, completing appropriate documentation that meets the Code of Maryland Regulations (COMAR).  The Health Services Nurse also actively participates in the quality assurance and utilization review process.  

 

Essential Duties and Responsibilities:

  • Monitors the health status of all participants, completes appropriate documentation that meets COMAR and notifies the Director of Health Services when changes are observed

  • Completes nursing notes on all participants as required

  • Accepts, writes and transcribes verbal orders from the physician and follows-up in written form with the physician by using the 90-day doctor’s order tracking sheet to monitor the timely return of orders

  • Identifies participant/family needs and incorporates into the participant’s Plan of Care

  • Communicates with caregivers/participants and other medical professionals as appropriate for the coordination of care

  • Assists with nursing case management by obtaining needed medical and dental appointments, and follows up as needed

  • Monitors clients for any necessary rehabilitative services (i.e. PT, OT, SLP) and initiates acquisition of services

  • Ensures that all communication with the internal and external community, whether written or verbal, is professional in nature and appearance

  • Provides direct supervision to Medical Day staff

  • Administers medications as required and conducts any instruction in medication, nutrition or activities of daily living to participants and/or families

  • Prepares assessments and plans of care for Delmarva Review and is responsible for assessments of potential clients to the Developmental Disabilities (DD) program

  • Provides direct attendant care, when necessary for the DD population

 

Additional Duties and Responsibilities:

  • Creates and organizes medical charts according to League standards

  • Completes vitals and weights monthly and PRN on all participants and documents on appropriate forms

  • Conducts training and educational in-services on various nursing and medical issues for staff and participants

  • Assists with the evaluation and maintenance of medical equipment owned by the Company or issued to participants including wheelchairs

  • Leads group activities as directed by the Program Director

  • Performs other duties as assigned

Qualifications and Education Requirements:

  • State of Maryland active and valid RN

  • Graduate of an accredited nursing program

  • Ability to maintain accurate records, prepare reports and evaluate and direct the work of subordinate staff

  • Ability to read and interpret government laws, regulations and related materials

  • 2 year’s nursing experience preferred, but will consider less experience for the right individual (New graduates are welcome to apply)

  • Knowledge of safety procedures and proper body mechanics

  • Knowledge of the needs of special populations
