Case Manager, Personal Supports


The Case Manager utilizes a person-centered approach to write Individual Plans (IPs) by establishing meaningful, measurable, and individualized goals for each participant in the Personal Supports Program.  The Case Manager will also monitor progress towards goals by completing quarterly IP Reviews and maintaining ongoing communication with the Manager and Direct Service staff. 

Essential Duties and Responsibilities:

  • Scheduling all Individual Plan meetings and interim meetings with Coordinators of Community Services and other relevant team members.

  • Complete 90-Day Reviews for each participant by reviewing progress towards goals and making recommendations as needed (augmenting specific measures, implementation strategies, etc.)

  • Maintain up-to-date Participant Profiles/Face Sheets and demographic information.

  • Provide quality case management services to meet participant needs as they arise (includes providing resources for ancillary services such as housing/residential, benefits, day care, mental health needs, etc.)

  • Maintain close communication with participant caregivers regarding participant progress and needs.

  • Serve as the primary Incident Report writer for the department, adhering to reporting timelines.

  • Other Duties and Responsibilities:

  • Assist Personal Supports Manager with general inquiries, tours, and intakes for potential participants

  • Assist Personal supports Manager with reviews of new applications for program participants to assure that The League can provide quality services according to each individual’s needs.

  • In conjunction with Director and Manager of Personal Supports Services, assist as needed with the development of DDA Service Funding Plans and/or add-on funding requests.

  • Serve as direct care staff during times of staff shortages.

  • Additional responsibilities as assigned by management.

Qualification Requirements:

  • Must possess basic computer skills including Microsoft Word and Outlook.

  • Must have excellent written and oral communication skills.

  • Must have exceptional organizational skills and the ability to work with autonomy.


Educational and/or Experience:

  • Bachelor’s Degree in a related field preferred. HS Diploma with at least 3 years of successful experience in case management and/or working with individuals with disabilities required.

  • Must have knowledge of, or the ability to quickly gain understanding of, DDA and DORS regulations.
