Reports to: Sr. Manager, Person-Centered Planning
Exempt/Non Exempt: Exempt
Summary: This position will also be responsible for managing the Personal Supports Program, including participant recruitment, staffing, program implementation and ongoing supervision.
Essential Duties and Responsibilities related to Personal Supports:

Recruit participants for Personal Supports by conducting outreach and marketing to DDA Service Coordinators, through Transition and Resource Fairs, and by working with other League department managers/directors to identify current existing participants who may benefit from Personal Supports.

Conduct intake/screenings for participants referred for Personal Supports.

Request Personal Supports funding, as needed, through DDA by writing and submitting a Service Funding Plan and Cost Detail outlining the specific request.

Work collaboratively with the HR department to recruit, screen, and hire quality staff to serve as Support Counselors in the Personal Supports program in an efficient manner.

Provide ongoing training and supervision to Support Counselors implementing Personal Supports.

Schedule and facilitate quarterly staff meetings to convene all Personal Supports staff for the purpose of information sharing, mandatory trainings, agency updates, DDA/funding updates, and general professional staff development.

Remain available for on-call/crisis situations, and to provide guidance and support to Support Counselors working remotely as needed.

Provide support in the field (community or participant’s home) when necessary (i.e., to assist with behavioral challenges, model strategies, observe staff interactions, etc.)

Complete monthly Personal Supports billing in PCIS.

Complete monthly Transportation billing forms, as instructed by DDA.

Review weekly timesheets to prepare excel spreadsheet for submission to HR/Payroll on a bi-weekly basis.

Review monthly Expense Reimbursement forms submitted by staff for accuracy, prior to submitting to HR/Payroll.

Monitor weekly progress notes to be sure all are present and spot check for quality, prior to filing in participants’ Master Files.

Participate in participant Person-Centered Planning meetings, including developing meaningful and measurable goals for participants receiving Personal Supports.

Attend trainings to ensure compliance with regulatory bodies and evidence based practices, incorporate into the program as appropriate.

Assure services meet CARF, DDA, OHCQ and other regulatory requirements for Personal Supports.

Qualification Requirements:

Access to a reliable automobile and a valid driver’s license with no more than 3 points.

Familiarity with DDA services and regulations.
-
Excellent written and oral communication skills.
Education and/or Experience:

Minimum of 3 years’ experience working with persons with developmental disabilities.

HS Diploma required, Bachelor’s Degree in Human Services, Psychology, or a related field preferred.
Minimum of 2 years’ supervisory experience.
